Hospice care is not only for the last days of life, and most hospices in South Africa do not turn people away for inability to pay. Referral early in a serious illness generally means better symptom control and more support for the family, not less treatment.

What hospice services usually include
- Pain and symptom management, which is the part families most often delay asking for.
- Home-based nursing care, so that a patient can remain at home where that is what they want.
- Counselling and bereavement support for the family, continuing after a death.
- Practical guidance on equipment, medication and what to expect.
Ask for a referral from the treating doctor, though most hospices also accept direct enquiries from families. Funding is usually a mix of donations, grants and a sliding-scale contribution.
Practical matters worth settling early
These conversations are difficult and are far harder left late. A living will recording a person’s wishes about treatment, and clarity about who may make decisions if they cannot, prevents families from having to guess during a crisis.
Locate the identity document, any funeral policy and the will in advance. A death must be registered through Home Affairs, and the death certificate is needed for everything that follows.
