There is no medical aid ombudsman in South Africa. The body that does the job is the Council for Medical Schemes, a statutory regulator with its own complaints unit, and it is stronger than an ombud — it can rule against a scheme and its rulings can be appealed through a formal structure.

Complain to your scheme first

The CMS will ask whether you have. Lodge the complaint with the medical scheme in writing, give it a reasonable time to answer, and keep the reference number. Only then does the CMS complaints unit take it. Going straight to the regulator usually gets you sent back a step.

This is where the CMS differs from an ombud. If you disagree with the complaints ruling, the matter can go to the Appeals Committee, and from there to the Appeal Board. Those are separate bodies within the statutory framework, so a decision is not the end of the road. Ask, in writing, what your appeal rights and time limits are the moment a ruling goes against you.

Prescribed Minimum Benefits are the usual reason to complain

Every registered medical scheme in South Africa must pay for Prescribed Minimum Benefits in full, regardless of your plan or whether your savings are exhausted. A scheme paying a PMB condition from your day-to-day savings, or refusing it because your benefits have run out, is the single most common valid complaint. The CMS publishes guidance on PMBs and on chronic benefits on its own site — read it before you write.

Medical aid is not medical insurance, and it matters here

The CMS runs a consumer education page called Medical Aid vs Insurance for a reason. It regulates medical schemes. A health insurance policy is a different product regulated as insurance, it has no PMB obligation, and the CMS cannot help you with it — that complaint goes to the National Financial Ombud Scheme instead. Check which one you actually hold before you write to anyone; the wording on the certificate will say.
