AMCU publishes a head office number and mailbox, and a full set of regional offices. Its own guidance is that members should not start at head office, and following that order is what actually gets a workplace problem dealt with.

No head office street address is published on the contact page. The union directs enquiries to the nearest regional office instead, and the regional list below is the part of the site that carries addresses.
The order AMCU asks you to follow
For a workplace problem, contact your shop steward or branch executive committee first, then your regional office if the branch cannot help, and only then lodge a formal complaint. The complaint form asks for the dates and times of the incident, who was involved, the name of the AMCU leader or organiser who assisted you, and the dates on which you reported the matter to the branch and to the region. Those dates are the point of the form: a complaint that skips the earlier steps has nothing to show.

Note that the West Rand office uses the gautengrgn address while Johannesburg Central uses jhbcentralrgn, which is the opposite of what the names suggest. Copy the address from the region you actually want rather than guessing from the province.
If the union itself is the problem
A member has rights against their own union as well as against an employer. If AMCU has failed to represent you or has not dealt with your complaint, the CCMA hears disputes between a union and its members, and the Registrar of Labour Relations at the Department of Employment and Labour oversees registered trade unions. Neither costs anything to approach.
