The provincial department forms part of the national health system, and clinics report through their district rather than directly to head office, so a clinic query normally starts with the clinic and then the district office. If a complaint about care is not resolved by the facility or the department, it can be escalated to the Office of Health Standards Compliance, whose Health Ombud investigates complaints about the standard of care in health establishments. Maladministration by a provincial department can also be taken to the Public Protector. Keep the facility name, the date and the patient file number, because a complaint cannot be traced without them.
