The Limpopo Department of Health does not publish a departmental telephone number of its own, and its old website no longer resolves. The route that does work is the provincial government switchboard in Polokwane.

The provincial portal lists eleven departments, Health among them, but publishes no separate number or mailbox for any of them. So ask the switchboard for the Department of Health by name, and say which function you need, because a query about a hospital, a query about employment and a query about a medico legal matter go to entirely different parts of the department.
One dead end worth knowing about. The department former web address at doh.limpopo.gov.za does not resolve, so a link to it from an older document or search result will simply fail. That is a lapsed domain rather than evidence the department has moved, and the working provincial portal is limpopo.gov.za.
Services and responsibilities
The department runs public health services across Limpopo, which means district hospitals, regional and tertiary hospitals, community health centres and clinics, emergency medical services, and the district health offices that sit between a facility and provincial head office.
Office and address information
Provincial head office is in the Mowaneng Building on Hans van Rensburg Street in central Polokwane. It is an administrative office, not a health facility, so nobody there can treat a patient, access a clinical record on the spot or admit anyone. For anything clinical, the facility is the place to go.
Applications and escalation
Work the ladder in order, because skipping a step wastes time. Raise a complaint at the facility first: every public health establishment is required to have a complaints procedure and a designated person to receive complaints, and most matters are resolved there. If that fails, the district health office is next, then provincial head office through the switchboard above. Get a reference and the name of the person you dealt with at each stage.
Beyond the province, the Office of Health Standards Compliance is the independent body that inspects health establishments against the national norms and standards, and complaints about the quality or safety of care at a facility can be lodged with it. The Health Ombud sits within that office and investigates serious complaints. A complaint about the conduct of an individual practitioner is different again and belongs with their professional council, the Health Professions Council of South Africa for doctors or the South African Nursing Council for nurses. National policy questions are for the national Department of Health rather than the province.
