Sizwe Hosmed Medical Scheme routes provider and member queries through separate mailboxes and numbers by function rather than through a single desk. The details below were read from the scheme’s contact page on 2 September 2026.
Task scope
This page covers the routes used when dealing with Sizwe Hosmed about treatment – claims, authorisations, referrals and the disease management programmes – rather than joining or changing a plan.
- Customer service: 0860 100 871
- General enquiries: queries@sizwehosmed.co.za
- Medical emergencies and ambulance: 082 911
Route
- New claims: claims@sizwehosmed.co.za
- Follow-up on a claim: queries@sizwehosmed.co.za
- Pre-authorisation: authorisations@sizwehosmed.co.za, confirm on 086 010 1176
- GP to specialist referral: primary.referrals@sizwehosmed.co.za
- Chronic disease and medication application: chronic@sizwehosmed.co.za, 086 010 3455
- CDL conditions: wellnessqueries@sizwehosmed.co.za, 0860 10 3455
- Oncology: oncology@sizwehosmed.co.za, 086 010 3454
- Maternity programme: bambino@sizwehosmed.co.za, 086 000 0048
- HIV programme: wellcare1@sizwehosmed.co.za
- Membership and cards: membership@sizwehosmed.co.za
Two things on the scheme’s page to be aware of. The referral telephone number is published as 0860 100 87 – nine digits, one short of a working number; the well-formed line on the same page is 0860 100 871. And the complaints mailbox sits on a hyphenated domain, sizwe-hosmed.co.za, while every other address uses sizwehosmed.co.za. That looks like a phishing pattern but is what the scheme itself publishes.
Requirements
Have the membership number, dependant code and the provider’s practice number. A claim is matched on the practice number, so a query without it usually cannot be traced.
Pre-authorisation must be obtained before a planned admission or procedure, not after. Sizwe asks that you email the request and then telephone to confirm it was received – the confirmation call is part of the process, not an optional extra.
Chronic and oncology benefits require registration on the relevant programme before the scheme pays. Registering late is a common reason medicine is rejected at the pharmacy.
Steps
Email the correct programme mailbox, telephone to confirm receipt where a number is given, and keep the reference. Sizwe also runs walk-in centres, including Johannesburg (23 West Street), eMalahleni (Plumer and Mandela Streets), Welkom (Buiten and Graaf Streets) and Durban, with a locator on its site.
Escalation
Complaints go to PO@sizwe-hosmed.co.za. Fraud is reported to an independently run whistleblower line – sizwehosmed@whistleblowing.co.za or toll-free 0800 212 150.
If the scheme does not resolve a dispute, a medical scheme member may take it to the Council for Medical Schemes, the statutory regulator, at no cost. Members are entitled to prescribed minimum benefits regardless of plan, and a rejected PMB claim is a common and legitimate ground for that escalation.
