POLMED provider contact number and provider login
The POLMED provider contact number is 0860 104 111. Doctors, hospitals and pharmacists use it for pre-authorisation and to register a member’s chronic condition. The authorisations department is also on fax 0860 104 114 and polmedauths@medscheme.co.za; members call 0860 765 633. Providers log in through the Provider Zone on the Medscheme portal, portal.medscheme.co.za, linked from POLMED’s Contact Us page.
POLMED member login
Members log in through the Member Zone at admin.medscheme.co.za/2013/Polmed/, linked from polmed.co.za. In the Member Zone you can see how your claims were paid, and POLMED’s guide encourages members to register on the website. A Telephonic and Internet Registration form is on the Forms and Downloads page.
POLMED provider locator and specialist network
To find a network provider, go to www.polmed.co.za and click Provider Search, or call 0860 765 633. You can also get provider lists through POLMED Chat on www.polmed.co.za or the WhatsApp line +27 60 070 2547. More than 3 500 specialists across all disciplines are on POLMED’s specialist network, and beneficiaries need a GP referral to see a specialist. POLMED has two options, Marine and Aquarium, and the Aquarium option uses a hospital network.
HearConnect is POLMED’s audiology network for the authorisation of hearing aids: call 010 880 6414 or email polmed@hearconnect.co.za for queries. The PPN optical network is on 087 210 0308.
POLMED chronic contact details
The POLMED Chronic Medicine Management Programme is on 0860 765 633, email polmedcmm@medscheme.co.za and fax 0860 000 320. Ask your doctor or pharmacist to register the chronic condition by calling 0860 104 111. POLMED pays chronic medication only after the condition is registered on the programme. Chronic medicines use a pharmacy network, with a 20% co-payment at non-network pharmacies. The Chronic Medication Indemnity form goes to polmed@medscheme.co.za or fax 0860 104 114.
POLMED medical aid rehab and mental health
POLMED’s Mental Health programme covers depression, bipolar mood disorder, PTSD and substance abuse. Email the programme at polpsych@medscheme.co.za. Admission to a hospital or facility needs pre-authorisation before the event on 0860 765 633 (members), or the R5 000 penalty applies. Through POLMED’s Psycho-Social Network, active serving SAPS members get 4 individual or 4 group post-trauma debriefing sessions a year.
POLMED PMB requests
POLMED’s Forms and Downloads page does not list a separate PMB application form. Send Prescribed Minimum Benefit queries to 0860 765 633 or polmedapmb@medscheme.co.za.
POLMED cancellation form
POLMED does not publish a cancellation or resignation form; its Forms and Downloads page has membership application, continuation, dependant and record-amendment forms only. Contact the Client Service Centre on 0860 765 633 or polmed@medscheme.co.za. Membership correspondence can be faxed to 0861 888 110. A member who resigns from SAPS, irrespective of years in service, does not qualify to remain a POLMED member.
What this organisation provides
POLMED, the South African Police Service Medical Scheme, is a closed medical scheme registered under the Medical Schemes Act as scheme number 374. Only SAPS employees appointed under the South African Police Service Act, and their dependants, can join. It has two options, Marine and Aquarium. Most of its service email addresses use Medscheme’s domain, medscheme.co.za.
Hospital procedures, treatment and admissions need authorisation before the event. If you do not get it, the guide says a R5 000 penalty applies. In an emergency, the member, a dependant or the hospital must tell POLMED within 24 hours or on the next business day. Emergency ambulance services must be authorised by ER24 within 72 hours, or a 40% co-payment applies. When you call for authorisation, have ready:
- your membership number and the patient’s name
- the date of admission or procedure, and the hospital
- the procedure and diagnosis, with the RPL and ICD-10 codes from your doctor
- the admitting doctor’s name and practice number

The main POLMED Client Service Centre is at Shop 17, Ground Floor, Nedbank Plaza, 175 Steve Biko Street, Arcadia, Pretoria 0083. Post for claims, membership and contributions goes to POLMED, Private Bag X16, Arcadia 0007.
The contact page lists regional walk-in offices in Bloemfontein, Kathu, Durban, Pietermaritzburg, Lephalale, Polokwane, Johannesburg (Mathomo Mall, Marshalltown), Cape Town, Port Elizabeth, Kimberley, Mbombela, Klerksdorp, Rustenburg, Roodepoort, Mahikeng, Vereeniging and East London. It gives no phone numbers for these offices, so use 0860 765 633. The 2026 guide also lists a Worcester office at 45 Church Street, Worcester, and additional service points at Durban Central SAPS, King William’s Town SAPS, Mthatha SAPS, Potchefstroom SAPS, Wachthuis Pretoria, Ulundi SAPS and Paarl East SAPS, with dates posted at the police stations. The website and the guide give different Polokwane addresses (Checkers Centre, Shop 3, 51 Biccard Street on the website; Checkers Centre, Shop 3, corner Hans van Rensburg and Grobler Streets in the guide). The two POLMED sources give different Roodepoort addresses: Valley View Office Park on the website and Flora Centre in the 2026 guide. Call before you visit.
POLMED Nelspruit and Johannesburg offices
The POLMED Nelspruit (Mbombela) walk-in office is at City Centre, Shop 11, corner Andrews Street and Madiba Drive, Mbombela 1200. Around Johannesburg, POLMED has walk-in offices at Mathomo Mall, 115 Main Street, Marshalltown 2001, and at Valley View Office Park, 680 Joseph Lister Street, Constantia Kloof, Roodepoort 1725. Neither office has its own phone number; call 0860 765 633.
Support and escalation routes
- Raise the query with the Client Service Centre on 0860 765 633, by WhatsApp, by email or at a walk-in office. Keep the reference number.
- If you are unhappy with the decision, send a written complaint or appeal to polmedappeals@medscheme.co.za, or post it to Private Bag X16, Arcadia 0007. Use the dispute form on polmed.co.za. POLMED says a dispute takes at least five working days.
