Old Mutual Funeral Claims Contact Details

Old Mutual takes funeral claims four ways, and one of them works without data or airtime: a free USSD code that starts the claim from any phone with just the deceased’s ID number. Where to submit a funeral claim

• USSD, free: dial *120*6672#, choose 1 to submit a funeral claim, then enter your SA ID number and follow the prompts
• WhatsApp: save 0860 933 333, send Hi, and choose the claims option
• Online: oldmutual.co.za/claims, following the claim submission prompts
• Email: contactus@oldmutual.com

For questions about the claim process, the guide gives 0860 222 274, or your Old Mutual adviser. The general service line, 0860 933 333, is open Monday to Friday 08:00 to 17:00 and closed on weekends.

Step one: the death must be reported

Old Mutual cannot pay until the death has been registered with Home Affairs. If that has not happened, the guide points families to Home Affairs on 0800 601 190. The Home Affairs death certificate and the DHA-1663 notification of death form (formerly BI-1663) are both needed for the claim.

Documents checklist

• The Old Mutual death claim form, downloadable from the claims page
• The DHA-1663 notification of death
• Banking details for the payment
• A letter of executorship or letter of authority, where the claim is paid to an estate
• A declaration by police form, where required

For a stillbirth or the death of a child, extra proof may be asked for: proof of stillbirth, parenthood, marriage or legal adoption, and proof of full-time study or disability for an older dependant. For cover above R50 000, Old Mutual says it may ask for more documents after its first assessment.
